The actual output shaft seal is GM Part 12547638, found by using the numbers UZ5050F off the original seal. The rear extension output shaft bushing is GM Part 15665313, found by someone's measurements in a comment on Amazon. Neither of these parts are supposed to go in my 2003 Silverado 1500 1/2 ton 4wd pickup.
