If you are having this issue and the dealer can not fix, it is because of faulty connections between small cables in the wiring system.

This got progressively worse, to the point where it wouldn't come on until you were five miles down the road.Problem solved today. 1. remove maplight assembly from the van by removal four 8mm screw/bolt, diconnect the harnesses. 2. flip over the assembly and push the map light base out. 3. push out to disconnect the map light switch from both side. 4. clean the socket connection and apply some bulb grease coating to the tips.
